Let's look at the forces reshaping the incident risk landscape right now.
Geopolitical Volatility as a Technology Risk
This is the dimension that most incident response plans from 2019 or 2020 simply do not account for. State-sponsored threat actors have moved from conducting targeted espionage against defense contractors to conducting broad campaigns against critical infrastructure, supply chains, financial systems, and mid-market companies that serve as upstream or downstream links in strategic industries.
The U.S. Cybersecurity and Infrastructure Security Agency (CISA) has issued repeated warnings about increased threat activity linked to geopolitical flashpoints. Organizations in manufacturing, logistics, healthcare, energy, and financial services have all been called out as elevated targets — not because they are high-profile targets individually, but because disrupting them at scale disrupts the broader systems they are embedded in.
What does this mean for your incident response plan? It means that your threat model needs to include adversaries with nation-state resources, persistence, and patience. It means that the attack vectors in your plan need to reflect the current tradecraft of sophisticated actors, not the signature-based malware of five years ago. And it means that your communications plan may need to account for scenarios where your incident is part of a coordinated, multi-sector event — where the usual external resources are already overwhelmed.
The Expanded Attack Surface of Cloud and SaaS Dependency
Most organizations have dramatically expanded their cloud and SaaS footprint over the past five years. This has created enormous operational flexibility, but it has also created a fundamentally different incident surface. When your data and workloads lived primarily in your own data center, an incident was largely bounded by your own infrastructure. B — Build a Living Framework
The most important conceptual shift in incident response is moving from thinking of your plan as a document to thinking of it as a capability. A document is static. A capability is practiced, updated, and integrated into the daily operations of your organization.
A living incident response framework has several characteristics that distinguish it from the traditional binder-on-the-shelf approach.
First, it is modular by incident type. Rather than one monolithic plan that attempts to cover every contingency in a single document, a living framework maintains separate playbooks for distinct incident categories — cybersecurity breaches, ransomware events, cloud/infrastructure outages, data integrity incidents, third-party vendor failures, and physical/environmental events. Each playbook is short, actionable, and designed to be used under pressure by someone who has not memorized it.
Second, it is role-specific and owner-assigned. Every section of every playbook identifies a named role — not a named individual — who owns that action. The incident commander is responsible for overall coordination. The communications lead owns all external and internal messaging. The technical lead owns the containment and remediation workflow. The legal and compliance lead owns the regulatory notification stream. Assigning to roles rather than individuals means the framework survives personnel changes.
Third, it is integrated with your actual current technology stack. This seems obvious, but it is the most common failure point. If your playbook references a backup system you migrated away from two years ago, or a SIEM tool that was replaced during a cost-reduction initiative, it is not just unhelpful — it actively misdirects your team during a crisis. The framework must reflect the real architecture of your real environment at the time of the incident, which requires regular synchronization with your technology roadmap.
Fourth, it is contractually anchored. Your framework should reference your current cybersecurity insurance policy (with specific notification requirements and timelines), your current retainer agreements with forensic response firms, your current SLAs with cloud and infrastructure providers, and any regulatory reporting obligations that apply to your industry and data types.

In many cases, these gaps would materially degrade their response capability at exactly the moment they need it most.
R — Rehearse with Realism
An incident response plan that has never been tested is not a plan. It is a hypothesis. And hypotheses fail in the most unpredictable ways.
Realistic rehearsal has three forms, and a mature incident response capability uses all three.
Tabletop exercises are structured, facilitated discussions in which key stakeholders walk through a hypothetical incident scenario in real time. A skilled facilitator introduces the scenario, injects complications and new information at timed intervals, and challenges participants to make decisions under the kind of ambiguity and time pressure that characterizes real incidents. The goal is not to execute the playbook perfectly — it is to surface the gaps, misunderstandings, and decision points that the plan does not adequately address. Tabletop exercises should be conducted at minimum annually, and more frequently after significant organizational or technology changes.
Functional exercises go a step further, activating actual response systems without causing real disruption. This might mean a simulated phishing campaign that tests real detection and response workflows, a controlled test of backup restoration procedures, or a simulation of cloud provider degradation using feature flags and traffic management tools. Functional exercises reveal operational gaps that tabletop discussions cannot — particularly gaps in tooling, access management, and technical execution speed.
Red team exercises are adversarial simulations conducted by an independent team (internal or external) that attempts to compromise your systems, exfiltrate data, or disrupt operations using realistic attack techniques. These exercises are the highest-fidelity test of your detection and response capabilities, and they consistently reveal gaps that neither tabletop nor functional exercises uncover. For organizations with significant regulatory exposure or complex infrastructure, red team exercises should be conducted annually at minimum.
The cultural dimension of realistic rehearsal is equally important. Exercises surface gaps, and gaps create discomfort. Leaders who treat exercise findings as opportunities for improvement rather than evidence of failure create organizations that get dramatically better over time. Leaders who suppress findings or deprioritize remediation create organizations that keep discovering the same gaps in real incidents.
A — Assess Your Risk Profile Continuously
A risk assessment is not a one-time event. The threat landscape, your technology architecture, your regulatory obligations, and your organizational footprint change continuously — and your assessment of the risks you face should change with them.
Continuous risk assessment does not mean conducting a full enterprise risk assessment every quarter. It means establishing processes that systematically feed new information into your risk picture on an ongoing basis. This includes:
Threat intelligence integration: Subscribing to and actively reviewing threat intelligence feeds that are relevant to your industry and technology stack. CISA issues alerts and advisories that are directly actionable for most organizations. Industry ISACs (Information Sharing and Analysis Centers) provide sector-specific threat intelligence. Your cybersecurity tooling vendors publish threat reports that identify emerging attack techniques targeting their customer base.
Technology change management hooks: Every material change to your technology architecture — a new SaaS tool, a cloud migration, a new API integration, a network redesign — should trigger a lightweight risk assessment checkpoint. This does not need to be a formal audit; it can be a structured discussion between your technology and security teams that asks: what new attack surface does this change introduce, and does it require updates to any incident response playbook?
Vendor and supply chain monitoring: Your risk posture is not bounded by your own perimeter. Your key technology vendors, cloud providers, and SaaS platforms represent part of your incident risk surface. Monitoring for news, advisories, and financial signals about your critical vendors allows you to anticipate and prepare for disruptions before they arrive at your door.
Regulatory landscape monitoring: Assign ownership for tracking the regulatory environment relevant to your industry. Data privacy laws, cybersecurity disclosure requirements, and sector-specific frameworks evolve, and your incident response obligations evolve with them. A legal or compliance team member should be responsible for flagging changes that affect your notification timelines, disclosure requirements, or mandatory reporting structures.
C — Communicate with Clarity and Authority
In the first hours of a major incident, communication failures are at least as damaging as technical failures. We have seen incidents where the technology team contained a breach effectively, but the communications vacuum created by the absence of a clear messaging strategy allowed speculation and rumor to reach customers, partners, and regulators before any authoritative message did.
A mature incident response framework treats communications as a first-class discipline with its own playbooks, pre-approved message templates, and clear ownership.
Internal communications need to operate on multiple tracks simultaneously. Executives need situation reports at defined intervals, even when the news is that there is no new news. Department leaders need guidance on what they can and cannot tell their teams, and what they should do if employees or customers ask questions. The incident response team itself needs a dedicated communications channel — separate from regular business channels, and secure — that is activated at the start of every incident.
External communications must be coordinated through a single authoritative voice. Whether that is your communications team, a retained PR firm, or your CEO, the principle is the same: one spokesperson, one message, one approval process. Pre-approved message templates for common incident scenarios — "we are aware of an issue and are investigating," "we have contained the incident and are assessing impact," "we have restored normal operations" — allow your team to communicate quickly without sacrificing accuracy or creating legal exposure.
Regulatory communications require their own track, owned by legal and compliance, with timelines that match your actual notification obligations. If your plan says "notify regulators as appropriate," it is not a communications plan. It is a placeholder. Your plan should specify which regulators, under which circumstances, within which timeframes, using which notification methods — and those specifications should match your current regulatory obligations, not the ones that existed when the plan was written.
Customer and partner communications require sensitivity to both relationship preservation and legal positioning. Your legal team should review customer communication templates before an incident — not during one. Knowing what you can say, what you should not say, and how to express transparency without creating unnecessary liability is a capability that needs to exist before the alarm goes off.
E — Evolve Through Every Incident
Every incident — even a minor one — is a learning event. Organizations that treat post-incident reviews as bureaucratic formalities lose the most valuable feedback signal available to them: real data about how their plan performed against a real scenario.
A structured post-incident review, conducted within two weeks of incident resolution, should address several questions with specificity. What did the plan say would happen, and what actually happened? Where did decision-making break down or slow down, and why? What information did the team need that was not immediately available? What tools or access were required that responders did not have? What communications succeeded, and which created confusion?
The answers to these questions drive targeted improvements to the relevant playbooks, to training and rehearsal plans, to tooling configurations, and to organizational structures. The organizations that improve most dramatically after incidents are the ones that treat the after-action review not as an exercise in assigning blame, but as a structured organizational learning process.
Evolution also means updating your framework in response to changes that do not originate from an incident. New threat intelligence, new regulatory requirements, significant technology changes, and the results of tabletop exercises all generate improvement items that should be tracked, assigned, and completed on a defined timeline. Incident response improvement is not an annual event. It is a continuous operational practice.
What Good Looks Like: Three Illustrative Scenarios
The following scenarios are composite illustrations — they reflect patterns we have observed across many client engagements, not the experiences of any single named organization.
Scenario One: The Healthcare Organization That Contained a Ransomware Incident in Hours
A regional health system with approximately 1,200 employees had invested in a living incident response framework two years before a sophisticated ransomware event targeted their administrative network. Their framework included a ransomware-specific playbook with pre-defined network segmentation procedures, a pre-established relationship with a forensic response firm (on retainer, not on speed-dial), and a communications playbook that included pre-approved language for patient notification, regulatory reporting, and media inquiries.

The Objections We Hear Most Often — and the Honest Responses
"We're too small to be a meaningful target."
This is the most common and most dangerous misconception in incident response. Sophisticated threat actors specifically target organizations that believe they are too small to be targeted — because those organizations typically have less mature defenses. The FBI's Internet Crime Complaint Center data consistently shows that small and mid-market organizations bear a disproportionate share of ransomware and business email compromise losses. Additionally, many incidents affecting smaller organizations are not targeted at all — they are opportunistic, exploiting widely known vulnerabilities against a broad population of organizations. Being small is not a defense. A current, tested incident response plan is a defense.
"We have cyber insurance, so we're covered."
Cyber insurance is risk transfer, not risk management. And increasingly, cyber insurance carriers are requiring documentation of specific security controls and incident response capabilities as a condition of coverage — and auditing those conditions at claims time. Organizations that discover after an incident that their plan did not meet their carrier's requirements have found themselves in coverage disputes during the worst possible moment. Beyond coverage, a well-executed incident response almost always results in materially lower incident costs than a chaotic one — and those costs, even when covered by insurance, affect your future premiums.
"Our IT team has this handled."
Incident response is not an IT function. It is an organizational function that requires coordinated action from IT, legal, communications, operations, finance, and executive leadership. The technical containment and remediation work is an IT function. The regulatory notification, the customer communications, the business continuity decisions, the media response, and the leadership coordination are not. Organizations that treat incident response as an IT responsibility discover their gaps in the most public and expensive way.
"We don't have the budget for this."
This is a real constraint, and it deserves an honest answer. The cost of maintaining a current, tested incident response framework is significantly lower than most organizations assume — especially when it is approached systematically rather than as a one-time project. A phased approach, starting with a gap assessment and the highest-priority playbook updates, followed by a tabletop exercise and remediation tracking, can be executed within a budget that most mid-market organizations can accommodate. The alternative — discovering your gaps during a real incident — is orders of magnitude more expensive.
"We did this a couple of years ago."
This is the most important objection to address. Doing this work once is not the same as having a current, tested incident response capability. The value of the work you did two years ago has been continuously eroding as your technology, your organization, your regulatory environment, and the threat landscape have changed. The question is not whether you have done this work — it is whether the work you have done reflects your organization as it exists today.
Your 90-Day Roadmap to a Living Incident Response Capability
For organizations that recognize the gap and are ready to close it, the following roadmap provides a structured starting point. This is not a comprehensive implementation guide — it is a prioritized sequence of actions that create momentum and early wins while building toward a sustainable capability.
Days 1–30: Assess and Anchor
Week 1: Locate and audit your current plan. Pull out whatever incident response documentation currently exists. Identify the last-reviewed date, the systems and vendors referenced, and the roles and individuals named. Create a simple gap log that tracks anything that is no longer current.
Week 2: Inventory your current risk surface. Document your critical systems, your cloud and SaaS dependencies, your key technology vendors, your regulatory obligations, and your cybersecurity insurance requirements and notification timelines. This does not need to be exhaustive — it needs to cover the top 20 percent of your technology and compliance footprint that represents 80 percent of your incident risk.
Week 3: Align with legal and insurance. Document these specifically — which regulators, which timelines, which notification methods.

Weeks 5–6: Update or build your highest-priority playbooks. Focus on the incident types most relevant to your current risk profile. For most organizations, this means a ransomware/malware playbook, a cloud/infrastructure outage playbook, and a data breach notification playbook. Each playbook should be role-specific, actionable under pressure, and no more than four pages.
Weeks 7–8: Assign ownership and establish governance. Identify the individual responsible for maintaining the incident response framework — not just executing it during an incident, but maintaining and improving it over time. Establish a quarterly review cadence with defined agenda items: threat landscape changes, technology changes, regulatory changes, and exercise findings.
Days 61–90: Test and Iterate
Week 9: Conduct a tabletop exercise. Design a scenario relevant to your highest-priority risk — a ransomware event, a cloud provider outage, a third-party data breach. Run the exercise with your leadership team and key technical stakeholders. Use a skilled facilitator who will push past comfortable answers and surface real decision-making gaps.
Week 10: Document and prioritize exercise findings. Create a structured finding log from the tabletop exercise, categorized by severity and owner. Build a 90-day remediation plan for the highest-priority findings.
Week 11–12: Complete quick-win remediations and schedule next exercise. Organizations with a culture of preparedness treat incident response as a strategic investment, not a compliance cost. Their leaders participate in tabletop exercises rather than delegating them. They ask about incident response improvement at quarterly business reviews. They celebrate the gaps that exercises surface rather than suppressing them. They communicate clearly to their teams that resilience is a core organizational value — and that the measure of resilience is not whether incidents happen, but how quickly and effectively the organization recovers from them.
